Processing Information:
- Your application will be hand carried to the Passport Agency in Washington, D.C.. There, the Passport Agent will examine the application and if all documents are in order, assign a date the passport will be issued, according to departure date from the U.S.
- SAME-DAY issue applications (which may be issued Same Day or within 24 hours) require proof of departure within four business days of being submitted to the passport agency and MUST be shipped to Passport and Visa Online using overnight Company.
- Other urgent applications can be issued as fast as three business days
REQUIREMENTS TO EXPEDITE U.S. PASSPORT RENEWAL
- Your current passport. The passport must have been issued no more than 15 years ago, and you must have been 16 years of age or older at the time your passport was issued. If you have a valid or expired Second Valid Passport, you must also include this passport.
- Passport Renewal Form DS-82, “Application for Passport by Mail.” This must be completed online athttps://pptform.state.gov/PassportWizardMain.aspx(“Apply for a Passport Book”). The application must be completed, printed out, signed and included in your package to PAVO. An original signature, preferably in blue ink, is required; no faxes, scans or copies will be accepted.
- Two NEW passport-style color photographs, taken within the last six months
- PAVO Letter of Authorization, completed and signed.The Letter of Authorization (LOA) allows PAVO to represent you to all agencies involved in issuing your passport. The signature on the LOA must be original, preferably in blue ink, no faxes, scans or copies will be accepted.
- Proof of Departure. Submit a copy of your flight itinerary, airline tickets, or a letter from your company (on company letterhead) stating that a passport is required for business travel. This letter must be signed by a representative of the company other than the applicant and must specify the applicant’s departure date(s) and destination(s).
- Proof of name change, if applicable. If your name has legally changed since your last passport was issued, provide the state-issued Original Document or Certified Copy showing the name change (i.e. Marriage Certificate, Divorce Decree, or Court Order). You must also submit a photocopy of your Driver’s License issued in your new name.
Lost or Stolen Passport

- Two Professional Passport photographs, taken within the past 3 months.)
- Proof of U.S. Citizenship: (one of the followings)
- a) An original or certified copy of your birth certificate, with a raised seal from the State Office of Vital Statistics. Hospital-issued birth certificates are not acceptable. You can acquire a certified copy athttp://vitalchek.com.
- b) naturalization certificate; certificate of citizenship;
- c) Consular report of birth abroad;
- d) Expired US Passport. NOTE: If your appearance has changed significantly and/or passport was issued under the age of 16, you must include in addition to birth certificate or naturalization certificate.
- Proof of Identity, with photo: a) valid driver's license, b) valid college identification card, c) current military or government i.d. card, d) a Certificate of Naturalization or Citizenship, e) a previous U.S. passport, f) corporate or employer i.d. card.
- Include proof of departure date required (copy of itinerary or air tickets showing applicant is departing the U.S., or letter from employer stating immediate international travel is required).

- Take the above requirements to your local acceptance agent for Passport Applications (usually a post office or county clerk or court house).Visit http://www.travel.state.gov/cgi-bin/search.accept.cgi to locate a Passport Acceptance Agent near where you live. Pay the clerk the U.S. government fee, including rush fees, and execution fees. Be prepared to write two checks for the total amount, as the clerk will retain $25.00 execution fee.
